The Center for Disability Services offers hope, innovation and achievement to the people we support.u00A0For 80 years, we have been one of upstate New Yorku2019s largest providers of programs and services for individuals who have disabilities.u00A0 Many of the innovative programs and vital services that we offer are not available elsewhere.We are looking for anu00A0Music Therapistu00A0to join our agency.Provides quality music therapy services: Provides individual and group music therapy IEP services to students in Cloverpatch and Langan programs, in accordance with each studentu2019s IEP. Supplements studentsu2019 program through 30-minute classroom music sessions, focusing on large group activities.In support of these services: Develops and implements an appropriate treatment plan based upon goals, physicianu2019s diagnosis, order or referrals, and the IEP of each student. Evaluates treatment outcomes and modifies treatment plans as needed. Provides Parent Progress notes as required by IEP. Develop formal IEP annual review for assigned students according to Cloverpatch/Langan policies and procedures. Develops formal evaluations, including goals, upon request of school district CPSE and County EI Maintains complete and accurate documentation of each session as required by the program. Maintains schedules and statistics as related to NY State requirements. Communicates and cooperates with physicians, program administrators, classroom team, parents, caregivers, school districts, and county service providers Attends multi-disciplinary and IFSP, CPSE and CSE meetings when directed.
